Diving Medical Statement
Prior to any in-water training, Bananarama in accordance with PADI standards requires all students to complete the following medical questionaire.
If you have any "Yes" questions to the questionaire, you must consult your physician before arrival to ascertain your fitness for diving. Be sure to obtain written proof from your physician that states you are fit to dive wth any previous or current condition and/or while taking any medication.
Physicians with extensive knowledge of diving health are available on Roatan if prior consultation is unavailable.
